ГОЛОВНА ЗАДАЧА  

Робити внесок у надання медичної та функціональної реабілітаційної допомоги пацієнтам. 

ЗАДАЧІ/ОБОВ'ЯЗКИ

Задача 1. Експертні знання та їхнє застосування:

1. 1. Визначати конкретні потреби, зосереджуючись на питаннях фізичної та функціональної реабілітації пацієнтів, спряти оцінці цих потреб та/або безпосередньо оцінювати ці потреби, за необхідності:

  • Оцінювати конкретні потреби пацієнтів та визначати індивідуальний план реабілітації;
  • Розробляти план втручання відповідно до результатів оцінки стану пацієнтів, що фокусується на повсякденній діяльності та сприянні участі осіб, які здійснюють догляд (опікунів);
  • Забезпечувати безпеку пацієнта під час втручання, а також чітко визначати застереження/протипоказання, пов’язані з різними захворюваннями, травмами та методами лікування, та їх обговорення з усім медичним персоналом, пацієнтом та особами, які здійснюють догляд за ним;
  • Нести відповідальність за технічну якість та актуальність реабілітаційних заходів, що проводяться.

1.2. Реагувати на виявлені потреби, надаючи безпосередні реабілітаційні послуги та за необхідності робити внутрішні перенаправлення, поважаючи конфіденційність та права пацієнта:

  • Базувати реабілітаційне втручання на відповідному клінічному обґрунтуванні та постановці функціональних цілей;
  • Надавати комплексні реабілітаційні послуги на основі інформованої згоди пацієнта та відповідно до його/її потреб;
  • Інформувати та навчати пацієнтів та осіб, які здійснюють догляд за ними (опікунів), щодо процесу реабілітації, включаючи елементи, які слід продовжувати робити самостійно у безпечний спосіб. Вказувати на ознаки, що можуть викликати занепокоєння, надавати інформацію про деand where to go for help;
  • Identify specific needs for assistive devices and/or mobility devices and make a donation in accordance with the principles of safe and functional use;
  • Make internal referrals to the appropriate department, within the framework of the existing internal referral system.

1.3. Demonstrate a willingness to improve professional competencies and abilities to improve the quality of care:

  • Conduct self-evaluation of your clinical practice and identify strengths and areas that need improvement for professional development;
  • Participate in practice reviews with colleagues with the goal of professional development.

1.4. Organize and plan rehabilitation activities

  • Responsible for daily planning of activities
  • Responsible for logistical planning of donations (request for the issuance of materials, acts of donation) in accordance with quality standards and according to the requirements of donors.

Task 2. Reporting and monitoring:

2.1. Provide data, follow-up cases and organize daily and weekly activities:

  • Complete initial assessment and discharge forms for each patient, as well as follow-up forms after each rehabilitation session conducted;
  • Contribute to regular reporting of technical activities in cooperation with the Senior Project Specialist;
  • Participate actively in the internal meeting of the multidisciplinary team to discuss the patient's journey and his recovery with colleagues from SEN and social workers.

Task 3. Impact:

3.1. Build and strengthen relationships with medical staff and coordinator in the community, following a defined strategy and with the support of the immediate supervisor:

  • Increase the awareness of the medical staff about the norms and standards of rehabilitation;
  • Positively influence the reputation of NI in the field of rehabilitation in the context of the mission.

REQUIRED COMPETENCES

Qualifications:

  • Bachelor's degree in physical therapy or occupational therapy.

Experience:

  • Clinical or hospital experience conditions
  • Work experience in the composition ofmultidisciplinary team
  • Clinical experience working with trauma patients
  • Prior experience working in humanitarian programs is preferred

Skills:

Professional skills:

  • Clinical management of rehabilitation of beneficiaries and their caregivers (guardians)
  • Ability to keep/record accurate patient records

  • Excellent communication skills and diplomacy
  • Ability to follow instructions and be a team member
  • Flexibility
  • High attention to detail
  • Good organization abilities
  • Practical, dynamic and problem-solving oriented

WORKING CONDITIONS

Contract:Employment contract (according to the legislation of Ukraine) Term

Type of employment:Full time
