Case manager for work with children and families in Nadiya i zhitlo dlya ditey v Ukrayini

The company "Hope and homes for children" is looking for a qualified and motivated Case manager for work with children and families to join our team. We are an international charitable organization that works in 10 countries around the world. Our mission is to give every child the opportunity to live in a safe and loving family environment. 

City: Boguslav (Myronivska, Kagarlytska, Rzhyschivska, Ukrainian, Obukhivska communities) p>

Working schedule: 9:00 - 18:00, five-day work week

Duties:

- Consulting social specialists and representatives of territorial communities to establish cooperation in identifying children and supporting families;

- Providing consultations and personal participation in the involvement of guardianship and care authorities in supporting children and families in order to prevent their placement to institutional facilities;

- Provision of services to 40 children and their families to prevent removal of children or to support those who are already in institutions based on the decision of the guardianship authority or at the request of the parents;

- Involvement of parents (persons who replace them) in cooperation to solve problems and stabilize the situation in the family;

- Assessment of the needs, situation, strengths and weaknesses of family members;

- Development and implementation of individual case management plans (case management);

- Monitoring and evaluation of the results of the services provided, their impact on the child;

- Documentation of the service provision process , compiling reports, reviews, survey results, etc.;

- Consulting and participation in the development of effective mechanisms of interdepartmental interaction and programs to support children and families at the community level.

Requirements:

- Higher education in the field of social work, psychology or related specialties;

- It is desirable to have trainings on working with children and families in the conditions of martial law;

- Experience in the relevant field is an advantage.
